INVITATION TO TENDER
The management of Irrua Specialist Teaching Hospital, wishes to invite Tenders from interested qualified contractors for the supply and installation of the following:

Whole blood bag centrifuge (Table Top), Coagulation analyser (6 channels), Ultra Sound Machine, Haematology Auto-analyser (5 parts), Automated Plasmerexpressor, Blood collection monitor, Elisa incubator, CL 100 Immunology Auto-analyser, Visible/invisible range spectrophotometer, medium size water bath, vortex mixer, Swinging bucket centrifuge, Electrolyte analyser, Automated Blood culture machine, Agarose Gel Electrophoresis unit set, Trans illuminator UV/light (GEL imaging system), Straight freezer- 80 degree, Refrigerator- 20 degrees, PCR Multiview plates. Thermal Seal (Sterile), Vortex lab mixer, micro pipettes (1,5,50 and 100 ul), Electronic weigh machine and Eppendorf tube rack.

1.0 ELIGIBILITY CRITERIA
(i.) Evidence of registration with the Corporate Affairs Commission (CAC)
(ii.) Detailed company profile showing evidence of relevant equipment and evidence of key personnel
(iii.) Tax Clearance Certificate for the last three (3) years 2014, 2015 and 2016 with TAX Identification Number (TIN). (3) years 2014, 2015 and 2016 with TAX Identification Number (TIN).
(iv.) Valid PENCOM Compliance Certificate
(v.) Evidence of Certificate of Compliance by Nigeria Social Insurance Trust Fund (NSITF)
(vi.) Valid Industrial Training Fund (ITF) compliance Certificate
(vii.) Interim Registration Report (IRR) as evidence of registration on National Database of Contractors, Consultants and services providers in line with Section 16(7) of the Public Procurement Act (PPA), 2007.
(viii.) Audited company account for the last three (3) years (2014, 2015 and 2016)
(ix.) Evidence of similar jobs completed/on-going by the company
(x.) Evidence of financial strength, credit worthiness and solvency from company’s Banker(s).
(xi.) Evidence of being a Manufacturer Representative for Medical Equipment.
(xii.) A sworn affidavit in line with the provision of Part IV, Section 16, Subsection 6 (f) of the Public Procurement act, 2007 stating that:
(a.) all Documents/Information presented are genuine and verifiable
(b.) one of the Director(s) of the firm has been convicted by any court of law within or outside Nigeria.
(c.) none of the Directors of the company itself is bankrupt
(d.) none of the officers of Irrua Specialist Teaching Hospital (ISTH) or Bureau of Public Procurement (BPP) is a former or present staff of the company
The documents are to be arranged in the order in which they are listed.
2.0 METHOD OF APPLICATION
The Bid Documents should be enveloped and sealed, stating the business name and address, the Lot no. on the reverse side of the envelope and addressed to:

Prospective Tenders are to pay non-refundable Tender Fee as specified above for the project to Federal Teaching Hospital Irrua using Remita platform and also visit the Hospital to obtain detailed description/specification from the Planning Department before bidding.
3.0 SUBMISSION AND OPENING OF BID
Technical and Financial bids MUST be in separate sealed envelopes and should be submitted on or before 11.00 am on Thursday 21st September, 2017 while the opening of bids takes place immediately after the closing time. Any bid not submitted before the closing time shall be rejected.
The hospital will not be responsible for any cost incurred by bidders in connection with this invitation.
