INVITATION FOR TECHNICAL AND FINANCIAL BIDS FOR EXECUTION OF APPROVED PROJECTS IN THE 2017 CAPITAL APPROPRIATION
(1.0) In compliance with the Procurement Act, 2007, the Federal Medical Centre, Abeokuta hereby invites reputable and competent firms with relevant experience and good track records to submit Technical and Financial Bids for 2017 Capital Projects in the following lots.
(2.0) LOT 1: Supply of Ultra Sound Machine
(3.0) LOT 2: Drilling of Boreholes and Reticulation of Water Systems
(4.0) LOT 3: Procurement of Stationery Waste Compactor Container
(5.0) LOT 4: Coordination of Sensitization Programme of Populace in the South-West Zone on Breast Cancer Scourge
INSTRUCTION TO TENDERERS
(1) Prospective bidders must show evidence of payment of non-refundable Tender Processing fee before collection of bidding documents
(2) Bids with validity period of less than 60days shall not be considered.
(3) Bids not properly marked “Technical” or “Financial” and not properly signed shall be rejected.
(4) For financial Bid to be considered, prospective bidders must be technically qualified by demonstrating qualification in the following areas:
(a) Evidence of registration with Corporate Affairs Commission (CAC).
(b) Copies of Memorandum and Article of Association.
(c ) Evidence of payment of Tax (Tax Clearance Certificate) for 3 years (2014, 2015 and 2016).
(d) Audited Accounts for 3 years (2014, 2015 and 2016) duly stamped by a reputable audit firm.
( e) Evidence of Bank’s support.
(f ) Certificate of Compliance with Pension Reforms Act 2004 issued by Pension Commission or evidence of Exemption.
(g) Certificate of Compliance with Industrial Training Fund amendment Act 2011 issued by the ITF or evidence of Exemption.
(h) Verifiable evidence of registration with the National Database of Federal Contractors, Consultant and service Providers by submission of Interim Registration Report (IRR) as evidence of compliance.
(I) Company Profile which should include;
(i) Names of current Directors as registered with CAC
(ii) Lists of Affiliated companies overseas with dealings on Tools and Equipment
(iii) Evidence of verifiable projects successfully executed on the subject matter of this procurement for the immediate past three years.
(j) Sworn affidavit indicating that:
(i) None of the Directors of the Company has been convicted in any court of law for criminal offence involving financial impropriety.
(ii) No Officer of the Federal Medical Centre Abeokuta is a former or present Director, shareholder or has any pecuniary interest in the company.
(iii) The Company is not in receivership, insolvent or bankrupt.
(k) Any other information that will further enhance the chance of shortlisting the company.
(6.0) COLLECTION OF TENDER DOCUMENTS
Tender documents are to be collected from the office of the Head of Procurement, Federal Medical Centre, Idi-Aba, Abeokuta between the hours of 8.00am and 4.00pm (Monday-Friday) upon presentation of authenticated payment details of a non-refundable tender fee of N25,000 (Twenty-Five Thousand Naira) only. Payments should be made at the revenue unit of the Federal Medical Centre, Idi-Aba, Abeokuta.
(7.0) SUBMISSION OF TECHNICAL AND FINANCIAL BIDS
(a) Documents for both Technical and Financial Bids shall be submitted separately in sealed envelopes addressed to
The Medical Director,
Federal Medical Centre, Idi-Aba, Abeokuta.
(b) Completed TECHNICAL AND FINANCIAL BIDS shall be submitted in two (2) hard copies and one (1) electronic copy (non-rewritable disk/DVD) in two (2) different sealed envelopes and labelled “TECHNICAL BIDS” AND ‘FINANCIAL BIDS” respectively with their company names and contact numbers written boldly on it and dropped at the Office of the Medical Director, Federal Medical Centre, Idi-Aba, Abeokuta between the hours of 8.00am and 4.00pm daily (Monday – Friday) on or before 12.00noon of Wednesday, 20th September, 2017.
(8.0) OPENING OF TECHNICAL AND FINANCIAL BIDS
Technical Bids shall be opened on the same day, Wednesday 20th September, 2017immediately after the close of submission at the Cardio Renal Hall of the Federal Medical Centre, Idi-Aba Abeokuta. Representatives of respondent Companies, N.G.Os, Civil Societies and the General Public are by this advertisement invited to witness the bid opening exercise. Financial Bids shall be opened after the Technical Bids have been successfully analyzed. The date and time of the opening of the financial bids shall be communicated to the successful companies in due course.
(i) Late submissions of Technical and Financial bids shall be rejected.
(ii) Bids submitted outside the prescribed procedure shall not be considered.
(iii) This invitation shall not be construed for commitment on the part of Federal Medical Centre, Abeokuta for contract award nor shall it entitle the Tenderer to make any claim whatsoever for indemnity from Federal Medical Centre, Abeokuta by virtue of having responded to this advertisement.
